Our hub offices service our communities in the following areas:
Metro South Hub (Greenslopes)
From our Greenslopes hub, we offer community support packages, SIL’s, and support to people living in (Level 3 – Supported accommodation settings). We work closely with the Woolloongabba Mental Health Services and the Princess Alexander Hospital to facilitate a smooth, coordinated and supported discharg back into the community for our customers. We are involved in our community and support Queensland Mental Health Week Community Events in Greenslopes.
Metro North Hub (Clayfield)
From leafy Clayfield in Brisbane’s inner north our Clayfield hub offer community support packages, SIL’s, and support to people living in (Level 3 – Supported accommodation settings). We work closely with the Fortitude Valley Mental Health Services and the RBW Hospital to facilitate a smooth, coordinated and supported discharge back into the community for our customers. We are involved in our community and support Queensland Mental Health Week Community Events in Clayfield.
Sunshine Coast Hub (Caloundra)
Supported Care’s favourite hub and home to our amazing SIL’s units. Our Caloundra hub offers community support packages, SIL’s, and support to people living in (Level 3 – Supported accommodation settings). We work closely with the Sunshine Coast Mental Health Team (Maroochydore) and Nambour Hospital to facilitate a smooth, coordinated and supported discharge back into the community for our customers. We are involved in the Sunshine Coast Community and love watching our customers thrive in the best place in Queensland.
