At Supported Care we recognise the challenges many people face within our community.

Supported Care’s Community Living Program strives to make living in the community as easy and accessible to as many people living with a psychosocial, intellectual, or cognitive impairment as possible.

The Community Living program is designed to bridge service gaps that exist for people living with a disability, such as the coordination of accommodation supports from start right through to being provided the supports you require in your home.

We aim to support you throughout your journey so that you can focus on living your best life.

Specialised TEAMS

Our specialised support workers are trained to help with your care needs including medication management and helping to prepare meals, but you determine the level for care necessary for you. 

Additionally, we can arrange for allied health appointments, GP visits, financial assistance or correspondence with any other support partners. 


The Supported Care staff are trained to assist with:

  • Daily support and care needs

  • Medication management

  • Implementing positive behaviour supports and de-escalation

  • Risk assessments

  • Organisation of activities

  • Transport

  • Coordinating and transporting to doctor and allied health appointments

  • Communication and organisation with a range of other support partners

Steps to your new home & supports

 

You are at the centre of finding you the right supports. We follow a multi-step process to ensure a smooth journey for you

Referral and Engagement

  • A senior manager will engage with you, our customer, to ensure we understand your needs

  • We will discuss the options available to you, based on your unique circumstances (including SIL and ILO options)

  • We will provide you with a recommendation on how to get you the best outcome

  • We will help you find the right housemate

Submission

  • After taking input from you and your formal and informal supports, we will submit an application with the NDIS

  • We will work with your supports to gather the evidence needed to support your application

Approval and Preparation

  • We will start training our teams to in your unique requirements; including tailored care, risk management, transition and other plans essential so as to ensure you are delivered the best supports to safely meet your needs 

  • We will begin introducing you to your team of support workers

  • The NDIA will provide you with an offer

Choosing Your New Home

  • We will help you choose a home that meets your needs

  • We can lease and sublease to you (including solutions for utilities, and or groceries where you require);

  • Where required, we will assist you to find the right flatmate

  • With all this in place we can plan for your move

Your New Life

  • We will assist you to transition to your new supports and home

  • We will provide you with ongoing high-quality supports that promote recovery and independence

  • We will continue to review and monitor your progress so that you can live your best life
