Link you with services and supports that meet your needs
Provide you with choice and control to ensure the experience is person-centred
Provide guidance and support in communicating with services so you don’t feel alone
Support you through the review process to ensure you get the best outcome from your planning meeting
We can link you with a Specialist Support Coordinator, Support Coordinator and/or Recovery Coach who can help you understand how each role can support you though your journey and provide the resources to help you lead a healthier, happier life.
What is Support Coordination?
A Support Coordinator assists you to understand your NDIS plan and connect you with funded and community services in your area.
There are three different levels Support Coordination:
Level 1 Support Coordination
This support will connect to services and supports that can assist you to reach your NDIS goals. This support is best suited to individuals who are confident in managing their own supports and NDIS plan.
Level 2 Support Coordination
This support will help you to understand your NDIS plan and connect you to services that are NDIS funded or community based. A Support Coordinator helps build your confidence, skills and capacity to establish, negotiate and maintain your services. They can help you maintain relationships with service providers and assist you at your NDIS plan reviews.
Level 3 Specialised Support Coordination
Specialised Support Coordination provides a higher level of support to people whose situations are complex and require specialist support. This support will help you manage challenges in your support environments and ensure that you are connected and maintain services
